Assessment

The first step to beginning treatment involves a comprehensive assessment that spans 1-3 sessions scheduled for 60 minutes or longer each. Patients stepping down from residential or intensive outpatient (IOP) treatment may want to schedule one 3-hour assessment to aid transition into outpatient services at AOBHC. Depending on treatment needs, the first several sessions may include an introduction to the practice policies and procedures, completion of a comprehensive clinical interview, completion of standardized measures, explaining the nature of the problem, and treatment planning to focus on a course of treatment.

The Importance of Completing
​Intake Forms and Measures

By completing the intake forms prior to your appointment, you will save valuable time during your session as your clinician can focus on reviewing only relevant and pertinent information for your assessment and treatment. At the end of your first appointment, your clinician will ask you to complete a set of evidence-based standardized measures to further evaluate your symptoms. These questionnaires are used to measure the severity of your symptoms and are given every 3 months to assess your improvement over the course to therapy. This ensures that your clinician is tailoring your treatment plan and delivering the interventions efficaciously. Sharing results from your assessments also help to motivate your engagement and participation in the treatment process.

Copies of Records

​Bring copies of any records you feel might be helpful, such as copies of neuropsychological evaluations, intake and/or discharge papers from previous services, or any behavioral observation and reports. You may request a secure link to upload the pdfs of your documents. Alternatively, you may fax the documents to (219) 228-1083.

Prior to your First Intake Appointment, you will be emailed and asked to complete the following:

1) Review and sign the Service Agreement

2) Set up the Patient Portal
3) Review and sign the Good Faith Estimate
4) Fill out the Intake Form
​5) Sign the Payment and Insurance Information Form

All forms must be completed by 9am the business day before your appointment. Those with Monday Intake Appointments must complete all forms by 9am the Friday before. This is to ensure timely processing of your information so that your clinician 
can focus on reviewing only relevant and pertinent information for your assessment. WE WILL RESCHEDULE YOUR INTAKE APPOINTMENT IF YOUR PAPERWORK IS NOT COMPLETED BY THE DESIGNATED TIME.
